Wavefront-guided LASIK using a high resolution aberrometer vs wavefront-optimized LASIK: a prospective study

Abstract Details

Purpose:

To prospectively compare outcomes between wavefront-guided LASIK using a high-resolution aberrometer and wavefront-optimized LASIK in the treatment of myopia. Outcome measures include high contrast snellen acuity, low contrast snellen acuity (5 and 25%), safety, predictability, efficacy and higher order aberration analysis.

Academic refractive surgical service Stanford University School of Medicine

Methods:

Ninety eyes of 45 patients were treated with wavefront-guided and wavefront-optimized LASIK. One eye was treated with wavefront-guided LASIK using the AMO iDesign aberrometer and the CustomVue excimer laser and the fellow eye was treated with wavefront-optimized LASIK using the Alcon Allegretto excimer laser. Eyes were randomized according to ocular dominance. Mean pre-operative spherical equivalent refraction was -4.67 +/- 2.28 and -4.47 +/-2.46 in the wavefront-guided group and wavefront-optimized group respectively. Mean cylinder was +0.71 +/-0.54 and +0.66 +/-0.68 in the wavefront-guided group and wavefront-optimized group respectively. There were no statistically significant differences in pre-operative HOA's between the two groups.

Results:

At post op month six, 85% of eyes in the wavefront-guided group and 79% of the eyes in the wavefront-optimized group were within 0.50 diopters of the intended correction. At the 6-month post-op visit the mean spherical equivalent refraction was +0.19 +/- 0.45 in the wavefront-guided group and 0.00 +/- 0.61 in the wavefront-optimized group. At 6-months, the mean cylinder was reduced to +0.11 +/- 0.18 in the wavefront-guided group and +0.17 +/- 0.25 in the wavefront-optimized group. At 6-months the mean total higher order RMS value was increased from 0.32 microns to 0.39 microns in the wavefront-guided group and 0.33 microns to 0.41 microns in the wavefront-optimized group.

Conclusions:

Wavefront-guided using a high-resulotion aberrometer and wavefront-optimized LASIK have similar clinical outcomes.
