To optimize energy and laser spot positioning parameters for femtosecond laser IOL transection.To present the surgical technique of femtosecond low energy IOL transection.

3 hydrophobic acrylic IOLs were transected with the energy 1,2,4,6,8,10 µJ and laser spot and layer separation size 7µm , light microscopy (IX-81, Olympus) was done to investigate the surface of the transected IOLs. The patient scheduled for hydrophobic acrylic IOL explantation. UCVA was 20/2000 and BCVA was 20/63. Before laser transection, the dispersive OVD (Provisc, Alcon, USA) was injected, the corneal incisions were watertight, the IOL was dispositioned out of the capsular bag. The femtosecond laser IOL transection was applied with 6µJ. The transected IOL was divided mechanically with two 25G end-gripping microforceps and the IOL fragments were explanted.


The excessive surface alteration was observed in case of 10,8 µJ parameters, at 6,4 µJ IOL surface was with medium alteration and with 2,1 µJ with minimal surface alteration. At 1-month follow-up, the patient’s UCVA and BCVA were 20/20 and no significant changes in endothelial cell density were apparent.


Low energy IOL transection can be applied in IOL explantation surgery for hydrophobic IOL with minimal IOL surface alterations and exfoliation of the undefined material The clinical application of femtosecond laser-assisted IOL fragmentation is feasible in cases of hydrophobic IOL explantation.
